yes, I got this about 40 years ago. I'm not sure how it all came about. I'm pretty sure my mother had something to do with it. I wish I hadn't covered my middle name up, but that's a story for another time. I wonder if this had something to do with the "Star Trek Welcommittee" that Bjo Trimble used to run way back when. if I remember correctly it showed up with two posters, one of Kirk and one of Spock, and three Starfleet insignia to be sewn onto shirts. the Captain, engineer and science officer.
this would have been about five or six years after the show went off air and was in syndication. it's hard to imagine now, with all the movies, tv shows and now remakes showing up, but then there was just the first series and the animated series that was out. and several books, two of which were "making of" or "behind the scenes" books.
it's a cool little piece of paper, and for years I actually believed it was signed by Gene Roddenberry and William Shatner posing as Captain Kirk. however, I recently did some research into their signatures and I've come to realize these are probably just secretaries who signed them. it's not like those actual signatures would ever be worth anything, right?
