Wow, was this a downer of an episode. I kind of figured it was going to be like this, so I wasn't surprised. Some of those first series What If...? stories had some very dark stories, but there were not that many. The main one I remember is the one with the villains taking over, the the Sphinx looking for the secret to his end. This is housed in the mind of someone on Earth, and he has to kill them to find it. Unfortunately, he had killed the person who had it before starting on his quest. I didn't care for that story much at all.
Once again, this story diverges from the main MCU in a way we never saw in the movie. Stephen is on a date with Christine ( hey, she was the first Night Nurse, how about that?), and she gets killed. This was never shown in the movie. The first episode followed Captain America right up to when Agent Carter got the serum instead of Steve. T'Challa got kidnapped instead of Peter. The last two just have made up scenarios to explain why they are different. With all apologies to Jim Cornette, that's LAZY BOOKING.
It was nice to see The Watcher Uatu actually interact with a character. That's a first. I liked the episode, but it's been my least favorite of the bunch so far.
